ECG Challenge: AF Patient With Lightheadedness and Rapid Pulse

A 73-year-old man with a history of paroxysmal atrial fibrillation and frequent premature ventricular contractions for which he is taking amiodarone presents to the emergency department with complaints of palpitations and lightheadedness that began 2 hours before presentation. His pulse is noted to be rapid, and a 12-lead ECG is obtained.
